Grey’s follows the lives of surgical interns, residents, and attending surgeons as they navigate surgical residencies, tackle complex medical cases, engage in groundbreaking research, navigate personal relationships, and grapple with legal or ethical dilemmas. For the majority of its run, the show primarily focused on Dr. Meredith Grey, portrayed by Ellen Pompeo, tracing her journey from the beginning of her surgical residency. However, new characters have been introduced over the years.

Season 20 of Grey’s Anatomy faces the challenge of transitioning away from Meredith as the central character, although she remains a part of the story. The season premiere, titled “We’ve Only Just Begun,” serves as a bridge between the classic Meredith-centric narrative cherished by fans and the new era established in Season 19.

Season 19 of Grey’s Anatomy maintained the intense drama that has defined the show. As the Interim Chief of Surgery, Meredith revived the residency program, welcoming a new group of interns and residents who are often considered second chances due to their previous setbacks in other medical programs. These individuals, including Simone Griffith, Lucas Adams, Benson “Blue” Kwan, Jules Millin, and Mika Yasuda, bring a fresh dynamic to the series.

The Season 19 finale, comprised of two episodes titled “Wedding Bell Blues” and “Happily Ever After,” brought the season to a close with a mix of love declarations, heartbreak, reunions, unexpected toothaches, and even a runaway bride. These two episodes also served as a transition point for longtime showrunner Krista Vernoff to hand over the reins to Season 20’s new boss, Meg Marinis. Vernoff directed the first part, while Marinis took charge of the second. Among the notable events in the finale, intern Simone leaves her fiancé at the altar, a group of beloved characters faces harrowing airplane turbulence, and many characters experience significant professional and personal breakthroughs.

Season 19 introduced a patient named Sam, whose health takes a drastic turn for the worse in “Happily Ever After” when he undergoes emergency surgery with Dr. Teddy Altman and a small team of interns. However, surgery encounters a major complication when Teddy collapses in the operating room due to a toothache, leading to a severe infection that causes her heart to fail. The finale concludes with Dr. Owen Hunt desperately trying to revive his wife as she flatlines alongside her patient. Additionally, the interns proceed with the surgery in the absence of an attending surgeon after Teddy’s collapse, resulting in the patient’s death on the operating table. Consequently, the Grey’s Anatomy Season 19 finale leaves Teddy’s fate hanging in the balance, along with the interns’ future careers.

It’s undeniable that fans harbor nostalgia for the classic era of Grey’s Anatomy. The MAGIC era holds a special place in their hearts, and the showrunners are embracing this sentiment by incorporating numerous callbacks to the show’s early seasons in “We’ve Only Just Begun.”

Legacy characters from Grey’s Anatomy, such as Meredith Grey, continue to receive significant attention. Despite bidding farewell to the show, Meredith still makes appearances in the episode, focusing on her ongoing Alzheimer’s research and the controversial theory she introduced in the Season 19 finale. This theory challenges decades of established research on the disease, including the legacy of her late husband, Derek Shepherd. “We’ve Only Just Begun” isolates Meredith’s research in a secretive bubble following her decision to speak out at the Catherine Fox Awards, where she clashes with Catherine Fox herself. Catherine’s antagonistic behavior, particularly during Richard Webber’s vulnerable moment at the award ceremony, further diminishes her likability. Unfortunately, Catherine’s villainous tendencies are not new to the show, and Richard’s susceptibility to relapse is a plot point that has been explored multiple times before.

The episode introduces parallels that are hard for fans to ignore. The interns face severe consequences after a patient’s death, reminiscent of the infamous LVAD wire scene from Season 2, where Dr. Bailey and Dr. Webber disciplined the interns. Additionally, “We’ve Only Just Begun” features an ambulance-related car crash as the trauma incident, echoing similar incidents from previous seasons. Surgeons operating in a confined space recall memorable sequences like the “heart in an elevator” from Season 2 and Meredith and Alex’s ambulance ordeal in Season 8. The reappearance of the hospital’s basement tunnels, where the original characters bonded, adds another layer of nostalgia. Notably, Dr. Marsh’s resignation as the Residency Director, placing Dr. Bailey back in charge of the interns, brings back the iconic “I have five rules” line, eliciting smiles from even the most seasoned Grey’s Anatomy fans.

The ambulance crash in “We’ve Only Just Begun” is technically classified as an MVC, or multi-vehicle collision, with the unique aspect being the involvement of a self-driving vehicle named Wayne. As the surgeons operate in the back of the ambulance, Wayne repeatedly collides with it, posing a significant danger to the medical team. This situation adds tension to the surgery, which is already challenging due to the mobile surgical site’s constant shaking. Interestingly, no one attempts to move the ambulance or stop Wayne until Blue intervenes by slashing the car’s tires to halt its rampage. Meanwhile, Yasuda confronts Dr. Jo Wilson for ignoring her 9-1-1 page to indulge in a romantic rendezvous with her best friend Link, culminating in a passionate makeout session and declarations of love.

“We’ve Only Just Begun” features several romantic scenes among Grey’s Anatomy couples, both past and present, although the romantic subplots may offer diminishing returns. While the show has explored problematic and dysfunctional relationships, it also celebrates beloved couples like Ben and Bailey and Nick and Meredith, whose future remains uncertain. However, romances such as Simone and Lucas and Jo and Link lack development and chemistry despite receiving attention.

The Season 20 premiere of Grey’s Anatomy, “We’ve Only Just Begun,” has its pros and cons. It continues the storyline from the Season 19 finale and provides answers to significant questions, such as Teddy surviving her toothache and none of the interns being fired. Additionally, Nick chooses to be with Meredith, and Bailey reclaims her authority with the interns. While the premiere may not be groundbreaking compared to previous seasons, it sets the stage for further development as the season progresses.
